What is a Community Health Needs Assessment?

As part of the requirements for not-for-profit hospitals, every facility is required to conduct a Community Health Needs Assessment every three years and put together a plan to meet the community needs that the assessment identifies.

Every community has different health care needs, including diabetes management, obesity and weight loss management, chronic illness, substance abuse, and more.

That's why our assessment team pulls together a broad set of health care consumers from each service area to help us identify the health needs of each individual community.

Rural health care is strengthened by partnerships

As a Critical Access Hospital in a rural setting, OSF HealthCare St. Francis Hospital & Medical Group partners with several health care providers and entities to offer the best care for our community and patients.

Areas of partnership include:

Emergency Services

A Level IV Trauma Center, OSF St. Francis Hospital emergency services partners with SCP Health to adequately staff the emergency department.

Physicians and advanced practice providers are on staff 24/7. Ground ambulance services are provided by Rampart and air ambulance services are provided by Guardian Flight (formerly Valley Med Flight).

Both partnerships are vital in getting patients to the right place, at the right time.

Additionally, local EMS services provide coverage in the remote areas of the Upper Peninsula. These volunteer services, including Rock, Masonville Township, Tri-Star, Baldwin Township and Delta County Search & Rescue, are necessary to provide emergency services for our communities.

The strength of these partnerships have contributed to the successful designation of a Level IV Trauma Center and ensures our community members receive the emergent care they need.

Cardiology

OSF HealthCare works closely with UP Health System - Heart & Vascular team to provide continuity of care to patients in Delta County.

UPHS cardiologists visit OSF Medical Group 3-4 days per week, providing care at home and eliminating the need for patients to travel to receive care. 

In the event that a patient requires more complex care, they can be referred to a member of the heart and vascular team in Marquette. 

Oncology

OSF HealthCare has a long-standing partnership with Green Bay Oncology.

The OSF St. Francis Hospital & Medical Group oncology clinic is staff five days a week with experts from Green Bay Oncology.

Patients can receive treatment, complete procedures and complete their visits in Escanaba from experts in cancer care.

Orthopedics

Advanced Center for Orthopedics physicians provide orthopedic care to patients in Escanaba, including office consultations and surgical procedures.

Patients can complete their care, including post-surgical rehabilitation services  without traveling outside of Delta County.

The OSF Sports Medicine and Advanced Center for Orthopedics teams work closely, ensuring patients are receiving the right level of care, from the right expert.

Hospitalist Services

Sound Physicians and OSF HealthCare partner to provide physicians and advanced practice providers dedicated to caring for patients in the hospital setting.
